Medical Emergencies

In the event of a life-threatening medical emergency, call medical emergency services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Baymeadows Healthcare and they will contact us.

After Hours Urgent Medical Needs

If you have an urgent medical need and need to talk to our doctors after our regular hours, please call our office at (904) 636-5400. A doctor is on-call 24 hours a day. Your call will be returned promptly.

Insurance / Payment / Billing

At Baymeadows Healthcare, we accept most major insurance plans. Please contact your insurance company or you may call our office at (904) 636-5400 to find out if we accept your insurance plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, debit cards, Visa, MasterCard, Discover, and American Express.

If you have a question related to your bill or insurance, please contact our billing specialists at (904) 380-2820, and leave a voicemail. Be sure to spell your first and last name and give us your account number, return phone number, and nature of your call. The billing representative will review your account and return your call within 24 to 48 hours. The billing office is available Monday-Friday 8 am to 4:30 pm.

Referrals

If one of our doctors suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible. Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. Please allow one week for the processing of routine referrals. Please call (904) 636-5400, extension 251 for assistance.

Medical Records Request

If you need copies of your medical records, we ask that you complete a Release of Record Form, which will also detail the fee associated with this service. This will ensure that your records will be copied as soon as possible. Please allow one week, from when you submitted your form and payment, to receive the copies of your records. We will contact you when your records are ready to be picked up.

If you have been referred to another doctor’s office and they would like to review any of your personal records, please have them mail or fax a request to us. We will send your records directly to the doctor as soon as possible.

Prescription Refills

Please contact your pharmacy 3 days before your prescription runs out. Your pharmacy will contact our office to expedite your refill request and notify you when you can pick up your medication. Phone calls directly to our office for refills may delay the process.

Hospital Affiliations

The doctors at Baymeadows Healthcare have hospital affiliations with Memorial Hospital of Jacksonville and St. Vincent's Medical Center. 

Missed Appointment Policy

To avoid a $30 missed appointment fee, we kindly ask that you call our office to cancel or reschedule at least 24 hours prior to your scheduled appointment time.

Uninsured Patients

We will see patients with no insurance. Please call us for more information.
